I think I would choose wealthy and ugly. If you are wealthy you would not have to worry about much things in life. Plastic surgery might not be so crazy when you can afford it easily. You could by fancy things and feel good that you are doing something that lets you buy them. So your self esteem would still be at a pretty good level.
